I've tried this once, but then everything in IVAN turned black and hurt my eyes...
So what program should I use and how am I supposed to use the colours?

You need a program that allow working with custom palette in PCX. You must keep the palette colors. The last colors are specials colors used by IVAN (the 16 last if I'm correct, look into the doc about colors for that).
Photoshop work with such files, but others too.

So far i've gotten by simply by cutting/copying pixels of the appropriate colours and moving them to form what I want

cool. I've edited IVANs graphics before but it wasn't that great, this might help.

And now I know who that guy in the .pcx is!

I did things in this way for a time too, 4zb4. I was terribly confused about the arrangement of the colour index until I finally found all the relevant pieces of information and compiled the document.
